Personal Health e-Chart is an electronic patient medical record that is not limited to internal hospital or clinic record-keeping, but is accessible to a user with internet access. The e-Chart keeps medical records that include patient medical histories, examinations taken and basic patient medical complaints. PHe also allows a user to monitor not only his/her own chart but also those of his/her dependents. PERSONAL HEALTH e-CHART COMPONENTS

Present Illness	is the data entry point wherein a user inputs his/her present health concerns. Included in this component are inputs of present body temperature, body mass index, and diagnoses by physicians.
Patient History	is a record of a patient's medical history such as family medical histories, allergies, immunizations, etc.
Physical Examination	contains records of examinations that the patient has undergone which includes detailed check-ups of every body part, adult examinations, and OB GYN Examinations.
Plan And Treatment	is a record containing data on a person's pending medical procedure.
Home Instruction	is a reminder of a patient's medical routine at home after either a check-up or discharge from a hospital. This component also serves as a reminder of scheduled follow-up appointments with a particular physician.
Prescription	is an electronic representation of a typical prescription record of a patient. This prescription covers detailed records of medicine prescribed to a patient such as drug generic names, dosages and frequency of use.
Medication Chart	is similar to the prescription component of PHe but concentrates on recording the intake history of the patient so that both the physician and patient can properly monitor a patient's drug intake.
